Piedmont Fayette Hospital in Fayetteville, GA is seeking a Home Medication History Information Technician to support indirect patient care by compiling the best possible home medication list at admission or as needed. The role works under the Home Medication Information RN and aligns with hospital policy.

Qualifications include a high school diploma or GED and 1 year of retail pharmacy experience; hospital and IV chemotherapy prep experience are preferred.

Competenze

• Education: H.S. Diploma or GED required.
• 1 year of experience in a retail pharmacy setting required.
• Hospital pharmacy experience preferred.
• IV admixture experience preferred.
• IV chemotherapy preparation experience preferred.
• LPT and RPI registrations required or Upon Hire; PTCB preferred.

Mansioni

• Assist with Home Medication History to ensure an accurate medication list at admission or as needed.
• Support RN and pharmacy staff to complete home medication information per policy.

Shift info: 7-on/7-off rotation, with shifts from 7:00 a.m. to 6:30 p.m. and every Monday and Wednesday off.

Education

• H.S. Diploma or General Education Degree (GED) Required

Work Experience

• 1 year of experience in a retail pharmacy setting Required
• Hospital pharmacy experience Preferred
• IV admixture experience Preferred
• IV chemotherapy preparation experience Preferred

Licenses and Certifications

• LPT - Licensed Pharmacy Tech Registration (current) with the state Board of Pharmacy as a Pharmacy Technician Upon Hire Required or
• RPI - Reg Pharmacy Intern Registration (current) with the state Board of Pharmacy as a Pharmacy Intern Upon Hire Required
• PTCB Certification Preferred
