Job summary

Piedmont Fayette Hospital is seeking a Home Medication History Information Technician to support the medication history process. This role works under the Home Medication Information RN to obtain accurate home med lists at admission or as needed, ensuring patient safety and smooth handoffs.

The position requires a high school diploma or GED and at least 1 year of retail pharmacy experience; hospital experience and IV admixture/IV chemo prep are preferred.

Qualifications

• H.S. Diploma or GED required.
• 1 year of experience in a retail pharmacy setting required.
• Hospital pharmacy experience preferred.
• IV admixture and IV chemotherapy preparation experience preferred.

Responsibilities

• Support home medication history process under RN guidance.
• Help obtain the best list of home medications at admission or as needed.
