Option Care Health, Inc. is seeking an Infusion Nurse II to provide intermediate direct patient care in the home or infusion suite under physician orders. This role requires an active RN license and ongoing commitment to patient safety and high-quality infusion services.

The position supports care coordination, teaching patients and families, and documenting visits in real-time. Travel to patient sites is expected, with supervision as needed.

Formación

• Active and unrestricted RN license in state of practice.
• Minimum of 1 year of nursing experience.
• Current CPR certification required.

Responsabilidades

• Initiates, develops, and implements intermediate nursing plan of care treatments and educates patients and families.
• Assesses patient needs and physical status at each skilled visit and documents changes.
• Provides training and mentorship to Infusion Nurse I’s as needed.
• Communicates with clinical team and documents care in real time for billing and care continuity.
• Coordinates discharge planning and prepares summaries for transferring care.
