Enliven Home Infusion Specialists Colorado is seeking an experienced Registered Nurse to service home infusion and infusion center patients in Fort Collins and surrounding locations. The RN will educate patients and families on prevention and healthy habits, provide hands-on nursing care, and coordinate with physicians and pharmacies to implement treatment plans.

The ideal candidate has strong Med/Surg/ICU/ER experience, current CPR/BLS, and eligibility for malpractice insurance.

Qualificações

• RN with Associate or Bachelor degree in Nursing.
• Minimum of five years of nursing experience preferred in an Acute Care setting.
• Strong Med/Surg, ICU, ER experience.
• Home Health/Infusion experience preferred.
• CPR/BLS required; ACLS and PALS preferred.
• Background checks, malpractice insurance, and drug screen required.

Responsabilidades

• Complete daily EMR forms for each patient.
• Provide admission, case management, and follow-up skilled visits for home infusion and infusion center patients.
• Administer ongoing care/infusions per physician orders and provide follow-up.
• Develop initial treatment plans with physicians based on orders and assessments.
• Coordinate with physicians and pharmacies to implement care plans.
• Educate patients and families on prevention and healthy habits.
• Document care plans, progress, and changes in records.
• Discharge patients after physician and manager approval; finalize documentation.
• Prepare visit/shift reports and update patient records; liaise with other disciplines.
