Job summary

Enliven Home Infusion Specialists Colorado seeks an experienced RN to service home infusion and infusion center patients in Cheyenne and surrounding locations. You will educate patients and families on prevention and healthy habits, provide ongoing care/infusions, and coordinate with physicians and pharmacies to ensure optimal patient outcomes.

Qualifications

• Minimum of an associate or bachelor’s degree in nursing.
• Background check, malpractice insurance, and drug screen required.
• Current CPR/BLS required; ACLS/PALS preferred.
• Five or more years of nursing experience preferred in an acute care setting.
• Home health/infusion experience preferred but not required.
• Strong IV placement and lines access skills.
• COVID-19 vaccination preferred by some partners.

Responsibilities

• Provide admission, case management, and follow-up visits for home infusion and infusion center patients.
• Administer ongoing care/infusions and follow physician/Clinical Manager directions.
• Develop initial treatment plans with physicians based on orders.
• Document care plans, patient progress, and discharge notes.
• Coordinate with physicians, pharmacies, and care teams.
• Educate patients and families on prevention and healthy habits.
• Complete daily EMR forms for each patient.
• Discharge patients with appropriate documentation.
• Prepare visit/shift reports and update patient records.
