Job summary

Apploi is seeking an experienced RN to service home infusion and Infusion Center patients in Apache Junction, AZ and surrounding locations. You will provide skilled nursing care, educate patients and families, and collaborate with physicians to ensure high-quality outcomes.

The role requires five years of nursing experience (acute care preferred), strong IV placement, and BLS/ACLS/PALS as available. A flexible schedule and travel may be involved to support patient needs and partner facilities.

Qualifications

• Licensed RN with an Associate or Bachelor’s degree in Nursing.
• Minimum five years of nursing experience; Acute Care preferred.
• Strong Med/Surg, ICU, ER experience.
• Home Health/Infusion experience preferred but not required.
• Current CPR/BLS required; ACLS and PALS preferred.

Responsibilities

• Complete daily electronic forms for each assigned patient within a secure EMR system.
• Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients.
• Administer ongoing care/infusions and provide follow-up as directed by Physician and Clinical Manager.
• Confer with physicians and pharmacies to develop the initial treatment plan from orders and patient assessment.
• Provide hands-on care, management, and evaluation of the care plan and patient education under physician supervision.
• Report patient care, conditions, and progress to physician, pharmacy, and Clinical Manager.
• Implement care plan with patient and family to achieve optimal outcomes.
• Discharge patients after consultation with physician and Clinical Manager, and complete documentation.
• Prepare medical documentation including case conferences, contacts, medication changes, re-certifications, progress updates.
• Prepare visit/shift reports and update patient records; consult with other disciplines for optimal care.
