Job summary

Amerita, Inc. is seeking a Pharmacist to join our Pharmacy team in New Jersey. The role reports to the Pharmacy Manager and operates from our Totowa branch, Monday–Friday with on-call evenings and weekends.

Qualifications

• Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
• Registered Pharmacist License in state(s) of practice.
• Minimum of two years in acute-care or one year in home infusion therapy.
• Experience with compounding infused drugs.
• Proven record of evaluating and monitoring drug therapies for home care patients.
• Excellent verbal and written communication skills.
• Flexible schedule with on-call evenings and weekends.

Responsibilities

• Evaluates and monitors drug therapy for home care patients with physicians, nurses, and other healthcare professionals.
• Adheres to policies, procedures, and state/federal laws governing pharmacy practice.
• Coordinates referrals and admissions with nurses, customer service, and distribution.
• Reviews and verifies new patient orders and documents them accordingly.
• Assists physicians and other professionals with home care planning and therapeutic objectives.
• Conducts initial and ongoing assessments to identify drug-related problems.
• Monitors outcomes by evaluating lab and clinical data.
