Accredo Specialty Pharmacy is seeking a Home Infusion Nurse (Specialty Medication Field Nurse) to deliver infusion therapy in patients' homes and collaborate with care teams. You will conduct assessments, manage documentation, and coordinate care while traveling within a large geographic region.

The role requires an active RN license and 1+ year in critical care, acute care, or home healthcare, with proficiency in IV techniques and a valid driver's license.

Qualifications

• Active RN license in the state of practice.
• At least 1 year in critical care, acute care, or home healthcare.
• Proficiency in IV insertion and infusion techniques.
• Valid driver's license and ability to travel within a large geographic region.
• Ability to schedule and perform multiple patient visits weekly, including some evenings/weekends.

Responsibilities

• Provide safe and effective administration of specialty meds (IV infusion) in patients' homes.
• Partner with pharmacists and care teams to ensure holistic patient well-being.
• Document assessments, treatments, and progress in patient records.
• Serve as the primary contact for patient updates and care coordination.
• Demonstrate autonomy in clinical decision-making to optimize outcomes.
