Job summary

Option Care Health is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or an infusion suite, under physician plans of care. The role requires a current RN license and at least 1 year of nursing experience, with CPR certification.

You will work under supervision and support multidisciplinary teams to ensure safe, compliant care. Responsibilities include initiating nursing plans, documenting care in real time, and mentoring less-experienced

Qualifications

Active and unrestricted RN license in the state of practice.
Minimum of 1 year of nursing experience.
Current CPR certification required.

Responsibilities

Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals.
Assesses patient needs and physical status at each skilled visit and documents changes for physician review.
Provides training and mentorship to Infusion Nurse I’s to ensure patient safety and compliance.
Communicates with clinical teams and patients to coordinate care and ensure timely documentation.
Performs discharge planning, safety checks, and maintains HIPAA-compliant records.
