Job summary

Option Care Health in California seeks a Clinical Pharmacist to provide safe and effective pharmacy services in the home infusion setting. This entry-level clinical role coordinates patient services with nursing, educates patients and caregivers, and ensures appropriate medication use in accordance with policy and regulatory standards.

The pharmacist collaborates to monitor patient progress toward goals, prepares and dispenses medications, calibrates equipment, and maintains compliance with DEA

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license in the State Board of Pharmacy.
• One of the following: sterile IV admixture; non-sterile compounding/immunizations and MTM; ASHP residency or APPE; or equivalent experience as approved by leadership.

Responsibilities

• Determines patient suitability for home care and collects information to determine home care eligibility.
• Designs, monitors and evaluates patient-specific pharmacy care plans for home care pharmacotherapy.
• Modifies care plans based on monitoring data and lab values; assesses pharmacokinetics and labs as needed.
• Provides medication-use education to staff, patients and caregivers; counsels on appropriate medication use.
