Job summary

Affinity Hospice Care Inc. seeks a compassionate Licensed Practical/Vocational Nurse (LPN/LVN) to provide direct patient care in home settings across California.

You will implement care plans, educate families, document care, and collaborate with the care team to support patients and their loved ones. This per diem role offers a rate of $50–$60 per visit, requires CA LPN/LVN license, CPR, and travel to patient homes with a reliable vehicle.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team - including OT, PT, HHA, and other disciplines - and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
