Job summary

Affinity Hospice Care Inc. seeks an experienced Licensed Practical Nurse (LPN/LVN) to provide home-based patient care under the supervision of the RN. The role emphasizes compassionate, patient-centered support in the comfort of patients' homes.

Per diem assignments pay $50–$60 per visit, with potential for full-time conversion based on performance. Travel to patient homes is required, and a California LPN/LVN license, CPR, and 3+ years' experience are essential.

Qualifications

Graduate of an accredited practical or vocational nursing program.
Active LPN or LVN license in California.
3+ years nursing experience; home health or med/surg background preferred.
CPR certification current with AHA.
Valid driver's license with insured vehicle and ability to travel to patient homes.
Strong communication and collaborative, patient-first mindset.

Responsibilities

Provide direct patient care in accordance with the state Nurse Practice Act.
Implement and follow individualized care plans established by the RN.
Educate patients and their families on self-care techniques and diagnosis-related information.
Document care accurately and in a timely manner.
Collaborate with the care team—including OT, PT, HHA, and other disciplines—and communicate patient needs to the RN or Clinical Supervisor.
Use supplies and equipment safely, adhering to aseptic technique.
Perform other duties as required for patient services.
