Job summary

WesleyLife is seeking a Hospice LPN Visit Nurse to provide compassionate, in-home nursing care to clients nearing end of life. You will assess and address symptoms, support daily activities, and collaborate with an interdisciplinary team to ensure comfort and dignity.

This full-time role requires travel to clients’ homes, strong communication with families, and the ability to work autonomously in the field while coordinating with RN Case Managers and social workers.

Qualifications

• LPN license required.
• Experience in hospice or end-of-life care preferred.
• Ability to work independently in the field while collaborating with a care team.

Responsibilities

• Travel to clients’ homes to provide comfort‑focused, person-centered care.
• Deliver hands‑on nursing care and symptom relief.
• Assist with daily activities and ensure physical and emotional needs are met.
• Monitor changes in condition and communicate with the interdisciplinary team.
• Support families with education, reassurance, and compassionate presence.
• Collaborate with RN Case Managers, social workers, aides, and spiritual care professionals.
