Job summary

Stella Maris Inc in central Maryland is seeking an Admissions RN for Home Hospice to join our compassionate team serving adults in the home setting. The ideal candidate holds a Maryland RN license and has hospice or home health experience, with EPIC experience preferred.

The role focuses on admitting patients to appropriate hospice programs, coordinating admissions, educating families and physicians, and providing on-call coverage as needed. A comprehensive benefits package is offered.

Qualifications

• Maryland RN license is required.
• Prior hospice or home health experience preferred.
• Experience explaining insurance benefits and patient responsibilities is a plus.
• Ability to coordinate admissions and develop initial plan of care.

Responsibilities

• Be the main contact for prospective patients and families about home hospice services.
• Explain insurance coverage and payment options to patients.
• Coordinate admissions and complete paperwork accurately and promptly.
• Develop initial care plan based on input from patient, family and physician.
