Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) to join our home health visit team covering Sudbury, Marlborough, Framingham, Natick, Northborough, Southborough and surrounding towns. You will provide skilled nursing and rehabilitative care to help clients remain at home.

Qualifications

• Massachusetts RN license in good standing.
• Graduate of an accredited nursing program.
• Minimum 1 year of adult/geriatric care experience in acute, home health, hospice, ICU or rehab.

Responsibilities

• Perform head-to-toe assessments and identify subtle changes in patient condition.
• Provide skilled nursing in the home, including wound care and IV therapy as needed.
• Triage urgent clinical situations and make real-time decisions.
• Maintain accurate, point-of-care documentation.
• Collaborate with multi-disciplinary team to ensure quality care.

BAYADA Home Health Care is currently seeking aRegistered Nurse (RN) to join our home health visit team. You will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to remain at home. How You’ll Make an Impact:

• Use strong head-to-toe assessment skills with the ability to identify subtle changes in patient condition
• Utilize your experience with acute and post-acute care (e.g., CHF, COPD, wound care, IV therapy, post-surgical care)
• Triage and manage urgent clinical situations independently in the home setting
• Perform medication reconciliation, teaching, and safety monitoring
• Perform comprehensive wound assessment, care, and documentation

Qualifications for a Registered BAYADA Nurse:

• A current nursing license in good standing in the state of Massachusetts
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
• At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings).
