BAYADA Home Health Care in Pembroke, KY is seeking a Full-Time Licensed Practical Nurse (LPN) to perform home health visits for adult and geriatric patients. Prior LPN experience is required, and the license must be valid in Massachusetts.

BAYADAHome Health has an immediate opening for a Full-Time LPN, Licensed Practical Nurse to perform home health visits**. This office provides exceptional home health medical visits to adult and geriatric patients in Pembroke, Marshfield, Hanover and surrounding towns**. Prior clinical experience as a Licensed Practical Nurse is required.

Licensed Practical Nurse (LPN) Responsibilities:

• Making home visits to clients living in designated territories.
• Performing assigned duties, including administration of medication, wound care, treatments, and procedures.
• Monitoring clients’ conditions; reporting changes to Clinical Manager and Client Services Manager.
• Following up with, executing, and properly documenting doctors’ orders.
• Performing client assessments as necessary.
• Case management and coordination.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records on a touch pad tablet.

Qualifications of a Licensed Practical Nurse (LPN):

• A current license as a Licensed Practice Nurse in the state of Massachusetts.
• Graduation from an accredited and approved nursing program.

Pay: Weekly

Base Pay, depending on qualifications and subject to negotiations

$42.00 - $48.00 per point
