Job summary

Heritage Home Health and Hospice in Idaho seeks a Speech Therapist to provide in-home therapy, coordinate care with families and other clinicians, and ensure high-quality patient outcomes.

You will follow agency and state policies, maintain accurate documentation, and participate in interdisciplinary meetings. Idaho licensure and CPR (BLS) are required; a valid driver's license and reliable transportation are essential.

Qualifications

• Be currently licensed as an ST in Idaho.
• A minimum of one year of clinical experience is preferred but not required.
• Demonstrate knowledge and skill in ST practice.
• CPR (BLS) professional certification.
• General knowledge of computers and the ability to learn industry-specific software and documentation applications.
• Possess a valid state driver’s license and automobile liability insurance.

Responsibilities

• Visit patients in residence and coordinate care with families and professionals.
• Be the main contact for all disciplines providing patient care and follow agency/state policies.
• Consult with the patient’s primary care physician, collect OASIS data (Home Health only), ensure medication profiles are accurate, and attend meetings as required.
