Job summary

Harmony Home Health Service seeks a licensed Speech Therapist to evaluate and treat patients at home in Albuquerque, NM and surrounding areas. You will implement therapy plans, collaborate with physicians, and educate family members on home programs.

Ideal candidates hold a Bachelor’s degree or higher in Speech Therapy, are licensed in the state, and have CPR certification. Home health experience is preferred and reliable transportation is required.

Qualifications

• Bachelor’s degree or higher in Speech Therapy.
• Licensed Speech Therapist in practicing state.
• CPR certification.
• Driver’s license and reliable transportation.
• Experience in home health preferred.
• Maintain patient confidentiality.

Responsibilities

• Provide treatment as ordered by the attending physician.
• Evaluate the home environment and make recommendations.
• Document patient care and progress in the patient’s record.
• Perform initial and ongoing assessments of patient functioning and communication.
• Develop and revise the plan of care with the physician and care team.
• Educate patient and caregiver about therapy plan and home programs.
• Supervise staff when appropriate.
• Participate in quality improvement activities.
