Job summary

Hackensack Meridian Health's Visiting Health Services of New Jersey seeks a Speech Language Pathologist to provide in-home therapy, evaluate patients, and develop individualized treatment plans in coordination with the health care team.

You will document progress in EMR, ensure timely evaluations, and educate patients and caregivers while meeting productivity goals and adhering to policies. Requires a Master's in Speech Pathology, licensure, and BLS/HCP certification.

Qualifications

• Master's degree in Speech Pathology is required.
• Minimum of six months Speech Pathology experience.
• PC experience/skills required.

Responsibilities

• Provide professional therapy to patients in their homes through evaluation, development and implementation of individualized goal oriented therapy programs.
• Complete patient evaluations within the specified time frame with holistic assessment.
• Develop and implement treatment plans with measurable short- and long-term goals.
• Prepare EMR documentation including progress notes, reassessments and discharge summaries.
