Job summary

Hackensack Meridian Health, in partnership with Visiting Health Services of New Jersey, seeks a Speech-Language Pathologist to provide home-based therapy to patients across New Jersey. Qualified candidates will have a MasterA0s in Speech Pathology and at least 6 months of clinical experience. The role requires licensure, strong documentation skills, and the ability to work independently in patientsA0homes. This position includes comprehensive benefits and tuition reimbursement.

Qualifications

• Master's Degree in Speech Pathology.
• Minimum of Six (6) Months Speech Pathology Experience.
• PC Experience/Skills.

Responsibilities

• Provide professional therapy to patients in their homes through evaluation, development and implementation of individual goal oriented therapy programs.
• Complete patient evaluations within the specified time frame with holistic assessment data.
• Develop individualized treatment programs based on evaluation findings with measurable goals.
• Prepare EMR documentation including progress notes, reassessments and discharge summaries.
• Coordinate with physicians and health care team to ensure effective care and appropriate referrals.
• Educate patients and caregivers with home-based programs and discharge planning as needed.
