Qualifications

• Master's degree in Speech-Language Pathology is required.
• 1+ year of clinical speech therapy experience is required.
• Recent Medicare-certified home health agency case-management experience is preferred.
• SLP licensure and Driver's License are required.

Responsibilities

• Document treatment provided in medical records and update care plans.
• Write modified orders for changes in the plan of care.
• Complete all documentation timely and accurately.
• Participate in clinical record reviews and therapy meetings as requested.
• Provide supervision to aides to ensure compliance with care plans.
