Job summary

Angels Care Home Health in Oklahoma invites a Speech Language Pathologist to join our Home Health team. The role provides rehabilitation services in patients' residences, developing and evaluating care plans and coordinating with an interdisciplinary team.

The position requires a master's degree, CCC-SLP, state license, and driving eligibility; two years of acute or rehab/home health experience is preferred. We offer comprehensive benefits and opportunities for professional growth.

Qualifications

• Master's degree in Speech Language Pathology from an accredited program.
• Current state license in Speech Language Pathology.
• Certificate of Clinical Competence from ASHA (CCC-SLP).
• Valid driver's license.
• 2 years' experience in acute care or rehabilitation, home health preferred.
• Reliable transportation with current auto liability insurance.

Responsibilities

• Provides coordinated care to patients in their place of residence.
• Plans, implements, and evaluates patient care plans to restore or maintain patient well-being.
• Collaborates with interdisciplinary team.

Education

Master’s degree in Speech Language Pathology

Certificate of Clinical Competence (CCC-SLP) from ASHA
