Job summary

LiveWell Homecare Agency, LLC seeks a licensed Speech-Language Pathologist to provide personalized, home-based therapy to patients with communication, swallowing, or cognitive challenges. You will work with a multidisciplinary team to assess, document progress, and plan care in patients' homes across California.

The role requires a Master's or Doctoral degree in SLP, an active license, and CPR certification, with at least one year of experience preferred.

Qualifications

• Master's or Doctoral degree in Speech-Language Pathology.
• Active SLP license in good standing.
• Minimum of one year of experience in speech therapy, preferably in home healthcare.
• CPR certification required.

Responsibilities

• Provide personalized, home-based speech therapy to patients with communication, swallowing, or cognitive challenges.
• Collaborate with a multidisciplinary home health team to assess, document progress, and achieve patient goals.
• Educate patients and families on safety, self-care, and therapy techniques; maintain compliance with state and federal regulations.
