Job summary

Interim Healthcare is seeking a Home Health Speech Language Pathologist to deliver comprehensive in-home evaluations and therapy. You will assess, plan, implement, and monitor speech, voice, language, and swallowing treatments in a home care setting.

You will collaborate with families and caregivers, document progress, and participate in care coordination and discharge planning while maintaining confidentiality and compliance with company policies.

Qualifications

• Meets ASHA CCC-SLP certification or equivalent.
• 0
• Current and unrestricted license to practice.

Responsibilities

• Evaluate, direct and provide speech/language pathology services to patients in the home.
• Participate in development and review of Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapeutic applications.
• Observe and report changes in patient condition to supervisor/physician.
• Provide instruction in alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff; maintain required documentation.
• Participate in care coordination and discharge planning.
• Maintain confidentiality and comply with company policy and training.
