Job summary

CenterWell Home Health is seeking a per diem Speech Language Pathologist to provide in-home evaluations and therapy to seniors and adults with chronic conditions. You will assess speech, language, and swallowing, design individualized plans, and deliver evidence-based interventions to improve communication and independence at home.

You will collaborate closely with an interdisciplinary team, document care in our electronic systems, and educate patients and caregivers on exercises and safety
