Job summary

Lakeview Village seeks a Speech-Language Pathologist to join its rehabilitation team, primarily serving Home Health patients. You will evaluate and treat speech-language disorders, consult with physicians, residents, and families, and coordinate with the interdisciplinary team for discharge planning.

Applicants must hold ASHA CCC or be eligible, and be licensed in Kansas. Strong communication and empathy for the elderly are essential in a team-based setting.

Qualifications

• Must have ASHA CCC-SLP or be eligible for it and be licensed in Kansas.
• Strong communication skills, empathetic approach, and ability to work in a team.
• Ability to follow documentation standards and maintain resident safety.

Responsibilities

• Perform evaluations and develop treatment plans for speech-language pathology.
• Consult with physician, resident, and family to set goals and plan therapy.
• Assess progress, adjust treatments, and coordinate with care team for discharge planning.
• Document all resident care services per procedures.
