Job summary

AdventHealth Home Health and Hospice Inc in Kissimmee, FL is seeking a licensed Speech Language Pathologist to provide therapy services in home health. You will evaluate, plan and implement treatment, educate patients and families, and collaborate with the care team to optimize patient outcomes.

Requirements include a Master's in Speech Therapy, SLP license, BLS, driver's license, and a passion for whole-person care. This is a full-time position with day and weekend shifts.

Qualifications

• Master's degree in Speech Therapy required.
• Knowledge of Home Care Regulations and third-party reimbursement.
• Ability to read, write and speak conversational English.

Responsibilities

• Observe and record clinical activities in the chart and report changes to physician or Case Manager.
• Perform evaluations to determine diagnosis, prognosis, and plan of care with physician input.
• Determine interventions, duration and frequency to achieve goals.
• Instruct team in speech therapy aspects of patient care.
• Supervise aides and ensure care plans and documentation compliance.
• Educate patients and caregivers on speech-language programs and home exercises.
