Job summary

Humana Inc. and CenterWell Home Health seek a Medical Social Worker to evaluate and plan personalized care for home health patients. You will lead the Social Worker Assistant and collaborate with the interdisciplinary team to deliver holistic services.

The role requires 1 year of health-care social work experience, licensure, a valid driver’s license, CPR, and strong discharge planning skills. You will document progress and connect clients with community resources.

Qualifications

• Minimum of one year of experience as a social worker in a health care setting (home health and/or hospice).
• Social worker licensure in the state of practice, if required by state law or regulation.
• A valid driver’s license, auto insurance, and reliable transportation.
• Proof of current CPR certification.
• Experience with discharge planning needs and obtaining community resources.
• Knowledge of medications and their correct administration.
• Ability to organize tasks, develop action plans, set priorities, and function under stress.
• Flexibility in work hours and travel locally.
• Maintain current licensure certifications and complete mandatory continuing education.
• Must read, write and speak fluent English.

Responsibilities

• Assess the patient’s social and emotional state in relation to illness or injury and care needs.
• Assess relationships between medical/nursing needs, finances, and community resources.
• Facilitate access to community resources to support recovery.
• Communicate with patients, families, and organizational colleagues.
• Provide ongoing assessment of patient and family needs and responses to treatment.
• Participate in care coordination as part of the interdisciplinary team.
• Evaluate Social Work Assistant performance to ensure compliant service delivery.
• Maintain and submit documentation including case conferences and progress notes.
• Use skills to make a meaningful impact in patient care.
