Job summary

Trilogy Home Healthcare is seeking a dedicated MSW or LCSW Social Worker to deliver high-quality home health visits across West Volusia County. This PRN/Per Diem role offers flexible scheduling and meaningful patient impact.

You will work under the Director of Clinical Services, coordinate with the clinical team, and leverage community resources to support patients. A Master's in Social Work or LCSW licensure is required, with 1–2 years of experience; home care experience is preferred.

Qualifications

• MSW or LCSW from an accredited school required for consideration.
• 1-2 years of experience required, with strong community knowledge.
• 1+ year of home care experience preferred.
• Ability to exercise initiative, sound judgement, and independent decision-making.
• Strong collaboration and communication skills.

Responsibilities
