Job summary

Trilogy Home Healthcare in Sarasota, FL is seeking an MSW or LCSW Social Worker for PRN/home health. You will provide compassionate, patient-centered care to home health patients and work under the Clinical Services leadership.

This role emphasizes using community, state, and federal resources to support patients, collaborating with the clinical team, and contributing to an award-winning, supportive, team-driven environment with flexible scheduling.

Qualifications

• MSW or LCSW from an accredited school.
• 2–3 years of social work experience with strong community knowledge.
• 1+ year of home health experience preferred.
• Ability to exercise initiative, sound judgement and independent decision-making.
• Strong collaboration and communication skills.
