Job summary

Trilogy Home Healthcare in West Volusia County is seeking an MSW or LCSW Social Worker for PRN home health visits. You will provide compassionate care to home health patients under the Clinical Services leadership, delivering care with patient focus and leveraging community resources.

• Strong collaboration and communication skills
