CenterWell Home Health in Reno, NV is seeking a Medical Social Worker to participate in interdisciplinary home health care. You will evaluate and plan care addressing emotional and social needs, supervise Social Worker Assistant, and ensure compliance with policies and standards.

Responsibilities include assessing patient social/emotional state, coordinating with families and care teams, and facilitating community resources to support recovery. Must maintain licensure and drive locally as needed.

Qualifications

• Minimum one year of experience as a social worker in health care, home health or hospice.
• Social Work licensure in the state of practice, if required by state law.
• A valid driver's license and reliable transportation.
• Proof of CPR certification.
• Experience with discharge planning and community resources.
• Knowledge of medications and proper administration.
• Ability to organize tasks, set priorities and work under pressure.
• English reading/writing/speaking fluency.

Responsabilités

• Assess the patient's social and emotional state related to illness/injury and care needs.
• Assess relationships of medical/nursing needs, finances, and resources.
• Facilitate access to community resources to aid recovery.
• Communicate with patients, families, and organization staff.
• Provide ongoing assessment of patient/family needs and responses to treatment.
• Collaborate with interdisciplinary team and advise on social work aspects.
