Job summary

Elara Caring is seeking a Speech-Language Pathologist to provide high-quality care in patients’ homes across our service area. You will assess, plan, and implement speech-language pathology services, ensuring safety and coordination with the care team.

Qualifications

• Master’s degree in Language Pathology from an accredited program.
• Current, unrestricted SLP license in the state of work.
• Minimum one year of clinical SLP experience.
• Ability to travel throughout the branch/office service area.
• Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
• Must be able and willing to travel 50%.

Responsibilities

• Promotes Elara Caring’s philosophy, mission statement and administrative policies to ensure quality of care.
• Evaluates the Speech-Language pathology patient to determine rehabilitation needs and develops a plan of care.
• Admits the patient and assesses speech-language therapy and all other needs according to OASIS and PPS regulatory guidelines; performs initial therapy evaluations and follow-up assessments.
• Initiates plan for patient safety using the patient, family, and community resources.
• Communicates significant findings to supervisor, physician, and team members involved with patient care.
• Participates in the implementation and development of the Plan of Care to ensure quality and continuity of care and proper discharge planning.
• Writes physician orders to cover additional visits and changes to the Plan of Care.
• Incorporates patient care goals into therapy, initiates and revises the Plan of Care as needed.
• Performs direct Speech-Language pathology services in accordance with accepted standards of practice and physician orders.
• Selects diagnostic and therapeutic materials appropriate to patient needs and instructs patients and caregivers in home therapy.
• Ensures therapy notes and logs are accurate and turned into the office according to policy.
