Job summary

Best Home Health Providers, Inc. is seeking a Speech-Language Pathologist to work in patients' homes across the Bay Area in a per diem role.

You will evaluate, treat, and support patients and families to improve communication, function, and quality of life. Requirements include a Master's or Doctoral degree in Speech-Language Pathology, California SLP license, CPR certification, and a valid driver's license with reliable vehicle.

Qualifications

• Master's or doctoral degree in Speech-Language Pathology.
• Active Californian SLP license.
• Minimum 1 year of clinical SLP experience.
• CPR certification with AHA.
• Valid driver's license and insured, reliable vehicle.
• Strong communication and organizational skills.

Responsibilities

• Evaluate patients and develop individualized care plans to address speech and language deficits.
• Deliver speech therapy using specialized techniques and equipment, and recommend communication devices when needed.
• Partner with referring physicians and the care team to ensure integrated, patient-centered treatment.
• Educate patients, families, and caregivers on treatment plans and goals.
• Participate in case conferences and care coordination meetings.
• Maintain accurate, timely clinical documentation per organization policy.
• Stay current in speech-language pathology practice and theory.
• Perform other duties as required for patient services.
