Job summary

Passion Care seeks a compassionate Registered Nurse (RN) to perform periodic home visits, conducting wellness checks and monitoring chronic conditions for home care clients. The role involves educating patients and caregivers and collaborating with the care team to deliver high-quality, person-centered care.

The RN will ensure accurate documentation, monitor vital signs, and update care plans as needed, while maintaining compliance with healthcare regulations and safety standards.

Qualifications

• Must hold a current and valid RN license in NJ.
• Strong clinical assessment and critical thinking skills are required.
• Excellent communication and interpersonal abilities are essential.
• Reliable transportation and a valid driver’s license are required.
• Ability to work independently and manage time effectively.

Responsibilities

• Conduct routine wellness check-ups and health assessments at patients' homes.
• Monitor and evaluate patient condition and vital signs.
• Review and update care plans based on clinical findings.
• Provide education and support to patients and families on managing health conditions.
• Collaborate with physicians, home health aides, and care coordinators.
• Maintain accurate documentation and timely reporting of visits.
• Identify and report any changes in patient condition that require medical attention.
• Ensure compliance with healthcare regulations and safety standards.
