Job summary

CenterWell Home Health is seeking a dedicated Home Health Registered Nurse to join a caring team, reporting to the Clinical Manager. You will deliver personalized, one-on-one nursing care in patients' homes, develop care plans with physicians, and help patients regain independence and return to the life they love.

The role emphasizes skilled nursing tasks, strong collaboration with a multidisciplinary team, adherence to safety standards, and thorough documentation.

Qualifications

Med surg, ICU, ER, or acute care experience.
Current and unrestricted RN licensure.
CPR certification.
Experience collaborating with a healthcare team.
Valid driver’s license and reliable transportation.

Responsibilities

Provide skilled nursing care in patients' homes, including meds administration and wound care.
Develop and implement individualized care plans with physicians and other healthcare professionals.
Monitor patients’ conditions and report changes.
Educate patients and families on disease management, medication, and treatment options.
Maintain accurate records of patient care and coordinate with other healthcare professionals.
Report patient care and condition progress to patient’s physician and Clinical Manager.
Oversee the work of Home Health Aides and monitor documentation.
