Job summary

Amedisys in Chesapeake, VA is seeking an RN Case Manager to perform patient assessments and coordinate home health plans of care for patients at home.

Requirements include OASIS knowledge, 1+ year RN home health experience, active RN license for the assigned state(s), CPR certification, and a valid driver's license. Weekend schedule options include Friday-Sunday or Saturday-Monday with competitive pay and robust benefits.

Qualifications

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities as assigned.
• Participates in clinical development and continuing education.

Attractive pay

• $80,000-86,000 Annually
• Salaried orientation period of 8-9 weeks

• Promotes patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicate patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.

Qualifications

• One (1+) year of clinical experience as a Registered Nurse (RN).
• Home Health experience
• OASIS documentation experience
• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
