Qualifications

• Minimum of 6–12 months of RN experience; home health preferred.

Responsibilities

• Provide skilled nursing care to adult and geriatric patients.
• Manage a steady caseload of approximately 5–6 visits per day.
• Perform patient admissions and develop individualized care plans.
• Deliver care focused on orthopedic recovery, wound care, and palliative care.
• Document care in real time using EPIC.
