Job summary

Avera Health is seeking an RN for home hospice nursing in the Sioux Falls area. The role involves providing in-home nursing care within a 40-mile radius of Sioux Falls and supporting patients and families with a collaborative multidisciplinary team.

The RN will perform assessments, develop and implement care plans, and evaluate patient progress while adhering to ANA standards. A sign-on bonus may be available and on-call duties are required roughly twice a month.

Qualifications

• Must hold an active RN license in the state of practice.
• Valid driver’s license and insurability required.
• Ability to communicate effectively with patients and team members.

Responsibilities

• Collects patient health data and analyzes assessment data.
• Develops a patient-specific plan of care and prescribed interventions.
• Implements and evaluates care to achieve expected outcomes.
