Job summary

Elara Caring in Garden City, KS is seeking a Registered Nurse Home Health to join our field-based care team. As an RN Home Health, you’ll assess patients, implement plans of care, educate families on care and safety, and monitor conditions during home visits.

Requirements: active RN license, nursing program graduate, 1+ year clinical experience; home health preferred. Must lift up to 50 lb and travel between homes.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.

Responsibilities

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy in accordance with HIPAA.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.
• Educating patients and families on their care and safety.
• Taking appropriate action when patient conditions change.
• Helping ensure quality, safe, and continuous patient care.
