Job summary

Enhabit Home Health & Hospice is seeking an experienced nursing leader to oversee the coordination and quality of home health services for patients. You will ensure care meets standards and regulatory requirements while collaborating with physicians, caregivers and staff.

Ideal candidates have RN licensure, a strong background in home health or hospice, and a track record of delivering high-quality patient care in a fast-paced environment.

Qualifications

• Graduate of an approved school of professional nursing.
• Must be licensed in the state where practice occurs (RN licensure); nurse licensure compact may apply.
• Minimum two years of nursing experience; home health or hospice preferred.

Responsibilities

• Coordinate overall home health services for patients per standards and procedures.
• Review and approve patient information during start of care, recertification, resumption of care, or evaluation visit.
• Facilitate relationships among physicians, referral sources, patients, caregivers and staff.
