Job summary

Pathwell Health in Winchester, VA seeks a Registered Nurse to assess patient health problems, develop and implement care plans, and maintain medical records in a home health setting.

You will collaborate with physicians and therapists, monitor patient conditions, provide education to families, and ensure accurate documentation. Requires Virginia RN license, CPR, and a valid driver's license; home health experience preferred.

Responsibilities

• Provide skilled nursing treatments as ordered by the client’s doctor.
• Complete evaluation tasks, including vital signs and medication reviews.
• Coordinate with physicians, therapists and other individuals in the care plan.
• Assess and chart observations of the client’s condition at each visit.
• Maintain complete and accurate documentation of nursing care.
• Work collaboratively with all members of the health care team.
• Provide education to clients and families on proper home health care procedures and strategies.
• Record patients' medical information and vital signs.
• Monitor, record, and report symptoms or changes in patients' conditions.
• Administer local, inhalation, intravenous, or other anesthetic.
• Prepare patients for and assist with examinations or treatments.
• Modify patient treatment plans as indicated by patients' responses and conditions.
• Order, interpret, and evaluate diagnostic tests to identify and assess patient’s condition.
• Monitor all aspects of patient care, including diet and physical activity.
• Engage in research activities and participation in formal educational training activities related to nursing.
• Perform other duties as assigned
