Qualifications

• RN licensure in state of practice.
• 2+ years of acute clinical or home health/hospice nursing experience.

Responsibilities

• Perform initial patient/home evaluation and determine safety for home health services.
• Provide skilled nursing care and patient/caregiver education according to plan of treatment.
• Develop and manage patient plan of care with physician orders and timelines.
