Job summary

Alpha Home Health & Hospice is seeking a full-time Home Health RN to support patients in the Skagit County area. Join a mission-driven team delivering one-on-one in-home care with autonomy and support.

Qualifications

• Washington RN licensure required.
• BLS certification required.
• Valid driver’s license and insured vehicle.

Responsibilities

• Conduct initial and ongoing patient assessments in the home.
• Develop and update individualized care plans with the care team.
• Administer medications, treatments, and injections as prescribed.
• Perform wound care and dressing changes and other complex care.
• Monitor vital signs and symptoms, reporting changes to the physician.
• Educate patients and caregivers on disease processes, medications, and self-care.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Document visits and care provided in the EMR.
• Ensure compliance with agency policies and regulations.
• Participate in case conferences and interdisciplinary team meetings.
• Provide emotional support and promote patient independence.

Home Health RN Responsibilities:
