PHC Home Health in North Carolina is hiring a Registered Nurse (RN) for PRN, part-time, and full-time shifts across Gaston and Lincoln County. The RN provides care at patients’ homes or assisted living facilities under physician direction, including assessments, treatments, and education.

Responsibilities include documenting per policy, coordinating care with the care team, and participating in weekly multidisciplinary meetings.

Qualifikationen

• Prior home health experience preferred.
• OASIS knowledge a plus.
• Excellent time management, organizational and reporting skills.
• Strong written and verbal communication.
• Current NC or multi-state RN license.

Aufgaben

• Provide nursing care including assessments, treatments, diagnostics, and patient education per plan of care.
• Observe patient condition, drug responses and report changes to supervisor or physician.
• Coordinate care with patient, physician, and other providers.
• Document accurately and timely per regulations and agency policy.
• Participate in weekly multidisciplinary team meetings.
• Engage in learning and job enhancement activities as directed.
• Participate in on-call rotation; one weekday on-call required.
