Job summary

Heritage Healthcare Services in Las Cruces, NM seeks a Registered Nurse to deliver skilled home health services under physician orders and plan of care guidelines. You will perform assessments, develop and adjust patient care plans, and provide counseling and education to patients and families in the home setting.

Responsibilities include coordinating with physicians, interdisciplinary teams, and external agencies, documenting findings, and participating in QAPI and staff trainings.

Qualifications

• A graduate of an approved school of professional nursing licensed in the practicing state.
• Minimum of six months’ experience as a registered professional within the last three years in hospital, nursing home, or home care.

Responsibilities

• Complete comprehensive OASIS assessments and ongoing interdisciplinary assessments.
• Develop and evaluate the plan of care with the patient and caregivers.
• Provide services ordered by the physician and educate the patient and family.
• Document clinical notes and communicate with physicians and other health care professionals.
• Coordinate with external agencies and participate in QAPI and in-service trainings.
• Update the plan of care in response to patient changes and physician orders.
• Supervise LPNs and home health aides as applicable; participate in on-call rotation.
