Job summary

Frontpoint Health is seeking a Registered Nurse (RN) for home health in the Dallas area on a PRN/per visit basis. The RN will manage visits within their licensure, including OASIS and supervised visits, coordinate care with the interdisciplinary team, patients, families, and referring agencies, and ensure timely documentation.

Responsibilities include case management, initial and ongoing patient assessments, medication administration, wound care, and patient education.

Qualifications

• Current Texas State RN license.
• 2+ years Home Health RN experience.
• CPR certification.
• Reliable transportation and valid Texas driver’s license.
• Current TB skin test.
• EMR experience (Kinsser preferred).

Responsibilities

• Case management and supervised visits.
• Perform skilled and comprehensive assessment of the patient during the initial admission.
• Assess and chart observations of the patient’s condition at each visit (vital signs).
• Provide skilled nursing care as needed in accordance with the plan of care (administering medication, cleaning/dressing wounds, assessing the healing progress).
• Provide proper patient education for home health care procedures and strategies.
• Responsible for the overall clinical record maintenance to include visit note (must be completed within 24 hours), comprehensive assessments, including OASIS (must be completed within 48 hours), medication profile, orders/plan submitted in a timely manner, etc.
• Maintain confidentiality of patient PHI through HIPPA law.
• Keep the patient comfortable through palliative care

Our Registered Nurse (RN) with have a variety of duties that will help the company thrive and manage continuity of care. This RN will see all visits assigned within the scope of practice of their license which includes OASIS visits, skilled nursing visits, and supervised visits. They will also participate in the compensated perception of new hires. Administer skilled nursing care for patients of all ages in their place of residence and to coordinate care with the interdisciplinary team, patients, families, and referring agency. Assumes the responsibility for, and participates in, the coordination of care.
