Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) for Home Health in St. Augustine, Florida. The RN will visit adult and geriatric clients, follow care plans, administer medications, and coordinate with the care team on a per visit basis.

Qualifications

• Current FL RN license.
• Minimum 1 year of clinical (medical/surgical) experience.
• Home care experience preferred but not required.
• Graduation from an accredited nursing program as indicated by transcript or diploma.
• Ability to travel to cases and work independently.
• Solid computer skills; experience with EMR preferred.

Responsibilities

• Follow a designated care plan per patient needs.
• Make home visits to clients in designated territories.
• Administer medications, wound care, treatments, and procedures.
• Monitor clients' conditions and report changes to the supervisor.
• Document doctors' orders and patient observations accurately.
• Perform client assessments and coordinate case management.
