Job summary

Aveanna Healthcare in Gainesville, FL is seeking a dedicated Registered Nurse for full-time Home Health. You will partner with patients and families to deliver personalized, at-home nursing care aimed at promoting recovery, safety, and independence.

Responsibilities include conducting in-home assessments, performing skilled nursing interventions, educating patients and caregivers, and coordinating with interdisciplinary teams to ensure quality outcomes and reduced hospitalizations.

Qualifications

• Active RN license in the state of practice.
• At least 1 year of clinical nursing experience.
• Valid driver’s license with reliable transportation and auto insurance.
• CPR certification (or willingness to obtain it).

Responsibilities

• Serve as the primary clinical partner for patients and families in home health, delivering one-on-one nursing care.
• Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs and translate findings into care plans.
• Provide skilled nursing interventions including wound care, infusions, catheter care, medication management, post-operative care, and chronic disease management.
• Educate and empower patients and caregivers with clear instruction and support for symptom and medication management at home.
• Monitor patient progress, identify changes early, and coordinate with physicians and the care team to adjust plans and prevent hospitalizations.
• Coordinate care across providers to ensure continuity and high-quality outcomes.
• Complete timely, accurate documentation reflecting the clinical picture and supporting quality and compliance.
