Qualificações

Responsabilidades

• Complete initial patient and family assessment to determine home care needs and plan.
• Regularly re-evaluate patient nursing needs and adjust plan of care.
• Coordinate care, administer medications as prescribed, and counsel the patient and family.
• Lead care planning with patient and interdisciplinary team; ensure discharge planning.
